Develops the complete assumption-to-prediction chain of the quantum measurement programme. The pilot-medium construction obtains a controlled total-variation limit on complete tagged Bell paths in physical time, together with material records on a fixed finite graph and horizon. The massive-configuration construction supplies a separate operational completion with guidance and initial equilibrium as premises, semibounded material apparatus, retained outputs and archive-history bounds. Earlier detector, preparation, protection and continuation theorems, proofs and counterexamples remain part of this integrated Version 2 publication. Neither construction follows from source/readout incompleteness alone.
